- OBTAIN EMERGENCY TREATMENT FROM YOUR EMPLOYER.
- OBTAIN A FORM 18, REPORT OF INJURY BY EMPLOYEE FROM THE NCIC INDUSTRIAL COMMISSION, OR DOWNLOAD IT
